Parental Consent Form
Hello!
With just a few days left to enter submissions to the contest, I just want to remind all the entrants that are under 18 that a parental consent form is needed with your entry. The form can be found online and can be scanned and emailed to [email protected] or faxed to 301-837-0312 (attention Mary Krakowiak).
